It must be a difficult job. Even if an artist feel bad, sad, depressed, low, angry, furious or sick, has to put a costume and a make-up on, go out on the stage and pretend. Perform.
Of course not necessarily but when writing it I thought of a belly dance show. Sometimes it may be quite difficult to express sadness through joyful music.

Not necessarily: one can use that energy as well, if it's emotions. A tad more difficult if you're feeling ill though.
